Start post off with where things are at.. ie : [ Warning / muted / jailed / kicked / temp/ban / Ip-Banned ]
Time event: 1hour ago from this being posted.
How long (ie for muted, jailed, tembanned: For a 1min mute, 5min jails, tempban 1day... etc
Reasons: Argumentative nuisance
+insulting etc
+trolling other players
+[insert 5hr chat log]
+[Insert stupid forum posts]
+[Anything else such as how many times was given warnings (include what for)]
+[How many times player was kicked if at all]
+[Any others who might be also be involved]

This is just a example (though based on some true events 
)
If you are on the server and some player is causing trouble, make a post with their name as the subject (keep all issues regarding same player in an existing topic that has been created (included any other known aliases)
Then just add any information regarding events, disputes, issues they have brought up... anything that could help other staff members check on to get a better picture.
You don't have to do these reports for small events (like new players who come on the server and most likely going to leave very soon) , you do have to do this for big events (usually involving players who have been on the server a few hours like 10+) and anything that is involving a player being muted, kicked, banned... warnings and temp jailing for minor stuff is ok.
Just use your own judgement if you think it would help other staff to know of any incidents that have occurred.
